WUHS had a vision of creating a cross-disciplinary teaching and delivery model to address the educational needs of its patients and the care needs of its community. It wished to create a plan in which team diagnosis and care was the standard of care for all patients not just specialized cases. Working with WUHS’s care teams, PMB created a 70-chair dental education floor and primary care clinics that filled a need in six of its medical education programs. The successful program anticipated the national trend for population health management and has become central to healthcare delivery in Pomona.
